Nursing care plans are a significant part of assessment in nursing education. I often hear nursing students (and even some faculty) state that you never use nursing care plans when you are working as a nurse. Nursing care plans however, are not so much about creating a written document to follow but about learning the nursing process. Assess, diagnose, plan, intervene and evaluate.

I have chosen to relate this activity to a cycle or a wheel. It is a never-ending process of assessment, prioritizing, intervening and evaluating until the patient is no longer in your care. This process is something that more senior nurses do automatically without even realizing they are “care planning”.

I have included a Prezi to illustrate the cyclical nature of the nursing care plan.
