I created a survey in MS Forms with my computer and phone.   The survey works well in both.  I liked the ability to set dependencies between the questions, and the suggested graphics have improved from what I’ve seen in the past.

It was simple and easy to use, and the results went into excel.

A feature that I appreciate is that I can have the respondents log in, which would be very useful with students.
